Assumption of Risk
I understand that participation in football officiating activities includes, but is not limited
to, classroom instruction, film review, physical movement, on-field drills, running, and
live football-related activities. I acknowledge that these activities involve inherent risks,
including but not limited to slips, falls, collisions, physical exertion, weather-related
conditions, and other potential injuries.

I voluntarily choose to participate in the HO Success Football Officiating Camp and fully
assume all risks associated with participation, whether known or unknown, foreseeable
or unforeseeable.

Release and Waiver of Liability
In consideration of being permitted to participate in the HO Success Football Officiating
Camp, I hereby release, waive, discharge, and hold harmless Hubert Owens, the HO
Success Football Officiating Camp, its directors, clinicians, staff, volunteers, sponsors,
and affiliates from any and all claims, demands, actions, or causes of action arising out
of or related to any injury, illness, damage, or loss that may occur as a result of
participation in camp activities.

This release applies to claims caused by negligence to the fullest extent permitted by law.

Insurance Responsibility
I understand that the HO Success Football Officiating Camp does not provide medical
insurance coverage for participants. I am solely responsible for providing medical
insurance coverage for myself or my child.​

Media Release

I grant permission for the HO Success Football Officiating Camp to use photographs, video recordings, or other media taken during camp activities for promotional, educational, or marketing purposes, without compensation.
